1. About the Practice
Curaidh Clinic is a private integrative medicine practice based in Dundee, Scotland. The clinic is led by Dr Minha Rajput-Ray, who is registered with the General Medical Council (GMC) and holds memberships with relevant professional bodies. The clinic is registered with Healthcare Improvement Scotland (HIS) as a private healthcare provider.

2. How to Contact Us
All routine enquiries, appointment requests, and administrative correspondence should be directed to the clinic office at office@curaidh.com. Where possible, please use the patient portal for appointment-related communications.
We aim to respond to all enquiries within two working days. Please note that the clinic office is not a medical emergency service.
If you are experiencing a medical emergency, call 999.
For urgent medical advice outside clinic hours, contact NHS 24 on 111.
3. Appointments
All appointments must be booked in advance through the patient portal or by contacting the clinic office. Appointments are subject to availability and are confirmed only after payment is received.
Please arrive on time for your appointment. If you are running late, please notify us via the portal or by email as soon as possible. Late arrivals may result in a reduced appointment time or rescheduling, at the clinic's discretion.
Please bring any relevant medical records, test results, or correspondence from other healthcare providers to your appointment. This helps Dr Minha provide you with the most informed care possible.

4. Fees and Payments
Full payment is required a minimum of 48 hours before your scheduled appointment. Payments are made by bank transfer to Curaidh Enterprises Ltd (Sort code: 40-16-57, Account number: 71513796). Please use your name as the payment reference.
Fees for additional services such as medical reports, letters, or referral correspondence may apply. The clinic office will confirm any such fees in advance.
5. Your Personal Data
Curaidh Clinic collects and holds personal and clinical data in order to provide you with care. This includes information you provide directly, as well as information obtained from other healthcare providers or testing companies where you have given consent.
Your data is held securely within our patient management system, Function365, and is handled in accordance with UK GDPR and the Data Protection Act 2018. We retain clinical records for a minimum of five years following the conclusion of your care with us, or as required by applicable professional guidance.

6. Sharing Your Information
We will not share your personal or clinical information with third parties without your express consent, except in the following circumstances:
- Where required by law or a statutory authority
- Where sharing is necessary to protect your life or the life of another person
- With our registrant body, the GMC, in connection with a complaint
- With contractors or advisers who provide services to the clinic, under confidentiality obligations
- With biochemical testing companies or other healthcare providers involved in your care, where you have consented
We may share anonymised case information with peers for professional development, clinical supervision, or publication. Your express consent will be sought before we do so.
7. Prescriptions and Referrals
Requests for prescriptions, referral letters, or other clinical correspondence should be made via the patient portal or by contacting the clinic office. Please allow adequate notice for these requests, as they cannot always be fulfilled immediately. The clinic office will advise on any associated fees and timelines.
8. Medical Records and Subject Access Requests
You have the right to request a copy of the personal and clinical data held about you. Requests must be made in writing to office@curaidh.com. We will respond within one calendar month of receiving your request and any information reasonably required to process it.
Fees may apply for the provision of medical reports or copies of records beyond routine subject access. The clinic office will confirm this in advance.
9. Chaperone Policy
You are welcome to bring a trusted person with you to any appointment. If you would like a clinical chaperone to be present during your consultation, please let us know when booking, and we will make the necessary arrangements.
10. Consent to Treatment
Dr Minha will discuss all proposed assessments, investigations, and treatments with you before proceeding. Your informed consent will be sought and recorded. You have the right to decline or withdraw consent at any time.
11. Complaints
We take all concerns seriously and aim to resolve them promptly and fairly. If you have a concern about any aspect of your care or the service you have received, please contact the clinic office in the first instance at office@curaidh.com.

12. Conduct and Communication
We are committed to treating all patients with respect and professionalism, and we ask the same in return. The clinic reserves the right to withdraw services in cases of abusive, threatening, or inappropriate behaviour directed at Dr Minha or any member of the clinic team.
13. Updates to This Policy
This policy may be updated from time to time. Any revisions will be published in the patient portal with an updated effective date.
